Case Manager

“When a new patient arrives, I visit them and identify what they’ll need when
they leave the
hospital. That could be anything from home healthcare, to Meals on Wheels,
to medication assistance. I try to keep in touch with my patients.
Sometimes –if
I haven’t seen them in a while– I’ll call up
just to see how they’re doing. Even
if it’s not part of my job, I like to keep in touch.”
Steve Copeland’s job is to make sure our patients get the attention they require
after they leave WNJ. He discusses needs with the patient, family and physicians,
then makes contact with appropriate public or private agencies. Aftercare can be
an important, even critical, part of healing. We want to be sure you know what you
need and how to get it, because healing doesn’t stop at the hospital door.
