What to expect on your first visit
When you arrive in the Therapy Services department, you will be greeted by our receptionist. He or she will ask you to fill out a few forms and then meet with our registration representative. This process may take up to 30 minutes, and is the reason we request that you arrive 30 minutes prior to your appointment or you can print the following forms and bring them with you to your first appointment, saving you a little time by already having the forms filled out.

At subsequent appointments, you will simply sign your name on the patient intake form and the receptionist will notify your therapist that you have arrived.
